A fourteen-year-old girl presented with a history of intermittent bilateral swelling in her ankles associated with moderate but progressively worsening pain that lasted for two years. The patient had been previously investigated and ankle X-rays, US and MRI, echocardiogram,
blood and urine tests were negative. Physical examination highlighted bilateral non-pitting oedema. A scintigraphy was performed. The present paper discusses different diagnostic hypotheses and treatment options.
