A four-year-old girl is evaluated for 8 recurrent episodes of perineal bleeding of undefined origin appeared six months before. Physical examination is normal except for a small anal fissure and a mild congestion of external haemorrhoids during Valsalva maneuver. Different diagnostic hypotheses and treatment options are discussed.
