Grayscale and Doppler modes are the imaging modalities of choice for the evaluation of patients presenting with acute scrotal pain. The informa- tion provided is vital to discriminate between situations that require immediate surgery, from those that are managed conservatively. It is therefore crucial to perform the study rapidly enough to avoid delay in surgery, when needed. Also, high-end equipment provided with sensitive Doppler modes is essential. The operator must be familiar with clinical presentation of the different scrotal pathologies and trained enough in optimizing Doppler parameters to obtain high-qual- ity imaging
