A case of renal cell carcinoma invading the surrounding organs and extending into the infrarenal vena cava, treated by enlarged nephrectomy, splenectomy, resection of the diaphragm and extended resection of vena cava, is presented. Special attention is paid to the diagnostic angiographic approach, to the surgical solution adopted and to the microvascular reconstruction of an injured aberrant vessel of the contralateral kidney.
